Missing Abbott Data

Hello!
Please follow the steps below to re-link and share your CGM data with Level2 so your Care Team can continue supporting you and the progress you’ve made.

Libre by Abbott App
  • In the Libre by Abbott app:
  • At the bottom right of the screen, tap Profile.
  • On the Profile page, tap Account.
  • On the Account page, tap Account Information.
  • On the Account Information screen, review the Email Address listed at the lower part of the screen – it may be difficult to read, but check it carefully to make sure there are no typos in the email address.
  • At the top left of the Account Information screen, tap the X
  • At the top left of the Account screen, tap the back arrow.
  • At the Profile page, tap Connected Apps.
  • Under the entry for Other Connections, tap Manage.
  • On the Other Connections page, you’ll see Level2 – tap the arrow at the right side of the screen. (If you do not see Level2 listed, skip down to Step 1 below).
  • Tap the red Disconnect option on this page – at the prompt Are you sure you want to stop sharing with Level2? tap the Disconnect option to confirm.
  • On the Connected Apps screen, tap Disconnect at the upper right.
  • A confirmation message will appear: Disconnecting will stop all data sharing with Level2 and your account – tap the red Disconnect
  • Open the Level2 Health App:
  • Tap at the bottom right on More.
  • At the top right corner of More, tap the Gear icon.
  • On the Settings page, tap Connect Devices.
  • If the FreeStyle Libre option shows Disconnect, go ahead and disconnect it and confirm that disconnect when prompted.
  • Tap Connect+ for the FreeStyle Libre.
  • You will need to use the email address you checked on back in Step 4 to sign in to the Libre account and agree to share your data. If you cannot recall your Libre password, feel free to reset it, and then return to Step 5.
  • After signing in to the Libre account, you should be presented with a form to agree to allow data sharing with Level2. Once completed, you should get a message that you are successfully linked.
  • Go back to the Libre by Abbott App.
  • On the Profile screen, tap Connected Apps.
  • Under the entry for Other Connections, tap Manage.
  • On the Other Connections page, you’ll see Level2 – tap the arrow at the right side of the screen.
  • You should see a new Connected On date for the Level2 account, with today’s date listed, so we know this a new connection we just made.
  • Level2 does not receive a live feed of your CGM data – after a linking, it can take a couple of days (sometimes longer) for the data to begin appearing on the Coach’s system at Level2.
Abbott Freestyle 2 App

 

  • In the Libre2 App:
  • Tap the Menu button with the three lines at the top left of the screen. (On Android phones, you will tap the three dots at the top right).
  • Tap Settings then Account Settings.
  • Confirm the email address listed is correct and free of typos – this same email address will be needed in a future step.
  • Return to the main page of the Libre2 app and tap the button at the top left, with the three lines.
  • Tap Connected Apps, and under Other Connections tap Manage.
  • You’ll see Level2 listed here, tap the arrow at the right side of the screen.
  • Note: If Level2 is NOT listed here, skip to the first step in the Level2 Health App.
  • Tap Disconnect and confirm the disconnect.

 

  • In the Level2 Health App:
  • At the bottom right corner tap the More tab and tap the Gear icon in the upper right corner.
  • At the Settings page, tap Connect Devices.
  • Click Disconnect for Abbott FreeStyle Libre and confirm Yes, you want to disconnect.
  • Tap Connect+ next to Abbott FreeStyle Libre
  • When prompted, log in to the FreeStyle Libre Account
  • Note: The email entered for the login must match the email address which was confirmed in Step C in the Abbott app.
  • Click ‘Allow’ and ‘Agree’ on the Share Data/HIPAA screens – once completed, a ‘Success!’ message should appear

 

  • Tap the button at the top, with the three lines.
  • Tap Connected Apps.
  • Under Other Connections tap Manage.
  • You’ll see Level2 listed here, tap the arrow at the right side of the screen.
  • You should notice an updated Connected On date – it should be the current date.

 

  • Level2 does not receive a live feed of your CGM data – after a linking, it can take a couple of days (sometimes longer) for the data to begin appearing on the Coach’s system at Level2.
Abbott Freestyle 3 App
  • In the Libre3 App:
  • At the top left of the screen tap the button with the three horizontal lines.
  • Tap Settings then Account Settings.
  • Confirm the email address listed is correct and free of typos – this same email address will be needed in a future step.
  • Return to the main page of the Libre3 app and tap the button at the top left, with the three lines.
  • Tap Connected Apps, and under Other Connections tap Manage.
  • You’ll see Level2 listed here – tap the arrow at the right side of the screen.
  • Note: If Level2 is NOT listed here, skip to the first step in the Level2 Health app.
  • Tap Disconnect and confirm the disconnect.

 

  • In the Level2 Health App:
  • At the bottom right corner tap the More tab and tap the Gear icon in the upper right corner.
  • Now on the Settings page, tap Connect Devices.
  • Click Disconnect for Abbott FreeStyle Libre and confirm Yes, you want to disconnect.
  • Tap Connect+ next to Abbott FreeStyle Libre.
  • When prompted, log in to the FreeStyle Libre Account
  • Note: The email entered for the login must match the email address which was confirmed in Step 3.
  • Click Allow and Agree on the Share Data/HIPAA screens – once completed, a Success! message should appear.

 

  • In the Libre3 App:
  • Tap the button at the top, with the three lines.
  • Tap Connected Apps.
  • Under Other Connections tap Manage.
  • You’ll see Level2 listed here, tap the arrow at the right side of the screen.

 

  • You should notice an updated Connected On date – it should be the current date.

 

  • Level2 does not receive a live feed of your CGM data – after a linking, it can take a couple of days (sometimes longer) for the data to begin appearing on the Coach’s system at Level2.
