Your participation in Level2 Specialty Care matters! Here’s how to keep your enhanced health plan benefits all year long.

Take one of the following actions every 60 days and you will meet the participation requirements:

  1. 1. Wear and connect a Level2-provided CGM
  2. 2. Complete a Level2 virtual care team or coach visit or coach chat
  3. 3. Sign in to the Level2 app

What happens if I don’t take one of the participation actions for 60 days?

  • Your UnitedHealthcare health plan benefits will revert to the standard plan for the remainder of the plan year. You will not be eligible to re-earn enhanced benefits until the next plan year. However, you may continue to receive Level2 Specialty Care support.
  • You are still eligible for Level2 Specialty Care and can re-connect with the Level2 team to work to improve your condition!

Level2 Eligibility Criteria

  • Be at least 18 years old
  • Have been diagnosed with type 2 diabetes
  • Enrolled in a select eligible UnitedHealthcare plan
  • Participation is limited to individuals who reside within the 50 United States. Residents of Puerto Rico, Guam, the U.S. Virgin Islands, and all other locations outside of the 50 states are not eligible
