When people join Level2 Specialty Care, they gain entry to the Level2 Health app, which includes access to:

A dedicated coach helps to set realistic goals and guides to success. Coaches (and their ideas, answers and encouragement) can be reached anytime in the coach chat.

 

Care team:

A coach is just one member of the care team and helps virtually connect members to other parts of the team, including physicians, nurse practitioners, nurses, registered dietitians or other type 2 diabetes experts.

 

Learning:

We bring proven, practical guidance from experts in diabetes, nutrition and behavioral health, designed to help people learn, make healthy choices and feel their best.

 

Daily actions:

Turning learning into action with daily actions, which suggest activities and reminders designed to bring you closer to achieving personal goals.
